This is the chemo regimine I will be starting next week.  This is a clinical trial out of the Mayo clinic.  Scared of what the side effects will be, if they will be tolerable.  this is a 21 day cycle.  Day 1 all three drugs, Day 8 all but avastin, week 3 rest.  then start the cycle over again.  mets to sternum and surrounding lymph nodes.  triple negative again.  ugh, why couldn't it have come back with some hormone receptors this time???  am hoping this chemo will knock it out.  my oncologist is not concerned about the liver lesions as they are very small. any advice for me?  what can i expect?  will this do the trick?  and put me back in NED???

This UM (University of Miami) originated  Nab-B-G regimen (nab-paclitaxel (Abraxane) + bevacizumab (Avastin) + gemcitabine (Gemzar)) is quite well-known, having first been reported in interim form at ASCO 2006, then updated and reported by the same UM team in peer-reviewed form in 2007, and finally and most recently at ASCO 2008. I consider it an enhanced and extended version of Kathy Miller's infamous E2100 regimen (paclitaxel + bevacizumab), enhanced to substitute the superior taxane, namely nab-paclitaxel (Abraxane) instead of paclitaxel (Taxol) and extended by the addition of gemcitabine (Gemzar), known to be additive or synergistic with each of the other agents independently.  To date, it's achieved very high ORR (overall response rate) in the first-line metastatic setting, including in triple negative tumors, with 76% of patients (14) achieving partial response (PR), and 6% (1) having stable disease (SD), for an ORR of 76%, and a CBR or clinical benefit rate (CR + PR + SD) of 80% in 17 patients evaluated for efficacy.  And this Nab-B-G regimen exhibit good tolerability and safety, with no grade 3/4 adverse events; the most commonly reported side effects were alopecia (62%), fatigue (38%) and rash (23%), and note that both nab-paclitaxel (Abraxane) and gemcitabine (Gemzar) can be dose-reduced if necessary.

can you explain that more and what it means?  is this good or bad? and can you tell me what alopecia is?  I have never heard that before. thank you for posting this.  am hoping that this is promising news for my situation.  don't really want to be doing this again but will do whatever i have to do in order to kick this cancer again.  thank you again.

Hi Cathi,
I will jump in for a sec....
76% had a partial response, meaning that the chemo had some effect on their cancer...6% became stable- meaning that their cancer stopped progressing completely.  Basically, it means that this combo has either a full or partial effect on 80% of the patients who are on it, either it stops it in it's tracks or it helps keep it from getting any worse.
It is a small study, of only 17, but 80% is very promising.

Alopecia is hair loss.

Good luck with this trial! I hope it kicks butt!!

hi Cathi, I'm so glad you've got a really good plan lined up. I had a great response on a clinical trial with avastin and abraxane.
